Tashkent, Uzbekistan (UzDaily.com) -- Protecting public health, strengthening work on prevention, early detection and diagnosis of diseases, in general, achieving efficiency in the medical system are among a number of priority tasks put forward by the head of the country in this area.
Large-scale reforms are being carried out in the republic to further improve the health care system, improve the quality and efficiency of medical services, and strengthen the health of the population.
In particular, the possibilities of providing the population with specialized medical care are expanding. Over the past four years, the number of specialized scientific and practical medical centers has increased from 13 to 17, 14 of their branches have been created in the regions, as well as 310 specialized places. The network of private medical organizations has been expanded, a competitive environment has emerged in the provision of such services.
The Committee of the Legislative Chamber of the Oliy Majlis on issues of public health has established parliamentary and deputy control over the implementation of these tasks.
On 18 September 2020, at a meeting of the Committee in the format of videoconference, information was heard from the head of the Republican Specialized Scientific and Practical Medical Center for Endocrinology (hereinafter - the Center) on the issue "On the state of providing endocrinological services to the population."
As noted at the meeting, our country has created a legal framework to streamline the sphere of endocrinology. In particular, the relevant laws contain norms regulating such services, decrees and resolutions of the President of the country have been adopted, aimed at ensuring the solution of issues in this direction. In particular, the National Program for Improving Endocrinological Assistance to the Population of the Republic for 2019-2021, approved by the resolution of the head of our state from 19 April 2019, serves as an important guide to action in this area.
A holistic, vertical integrated system of endocrinological assistance to the population has been created within the framework of state programs for reforming the sphere.
According to the data, 425.0 million diabetics are registered in the world. According to the analysis of the International Diabetes Federation, by 2045 it is planned to reach their number up to 629 million people. Globally, more than 665 million people suffer from thyroid diseases, and 1.5 billion people remain at risk of developing iodine deficiency diseases. The increase in such diseases is 5 percent per year.
In our country, the first place for such diseases is taken by diseases of the thyroid gland (goiter), the second is diabetes mellitus, the third place is diseases of the sex glands. Of these, diabetes mellitus also has social and economic importance, which is increasing along with other countries in Uzbekistan. In 2019, the incidence increased 3.2 times compared to 2001, by 11.4 percent compared to 2018, thyroid diseases in 2019 increased by 0.24 percent compared to 2018.
As noted in the information, the standards for diagnostics and treatment of endocrine diseases at all stages of healthcare have been updated, by opening specialized departments of endocrine surgery, intensive care units, diabetic foot, nephropathy, laboratories, operating units in the regions, the scale of such services is expanding.
Also, according to the decree of the head of our state, endocrinological dispensaries in the regions are provided with high-tech equipment in the amount of USD 2,324.0.
The center has established a system for providing on-site consultations with the participation of leading experts. At all stages of higher medical education, teaching hours for the study of diabetes mellitus have been increased, specialized courses have been organized for doctors of primary health care institutions.
The center has signed memorandums with 24 leading research centers in Asia, Europe and America. From 2017 to the present, 192 specialists have improved their skills in leading foreign clinics in countries such as Russia, Germany, Switzerland, Korea, Turkey, France, India, China, and the United States.
In the Center in the field of attracting foreign direct investment within the framework of cooperation with the Italian company "Vita Nova Diagnostic", the International Atomic Energy Agency attracted foreign direct investments for 1.5 million US dollars and 525,000 euros.
High-tech medical equipment was delivered to the Center on the basis of a public-private partnership with the attraction of foreign direct investment.
During the coronavirus pandemic, certain work has also been carried out in providing patients with specialized endocrinological care. In particular, the staff of the Center developed a Protocol for the management of hyperglycemic patients according to the severity of COVID 19 and for various acute and infectious diseases, which is distributed to all relevant medical and preventive and higher educational institutions. Also, online and audio consultations have been established in all regions for endocrinological patients infected with Covid-19.
At the same time, studies and analyzes have shown the need to strengthen work to improve the quality and capabilities of providing the population with specialized endocrinological care, to further strengthen the prevention of endocrine diseases and measures for their early detection, to widely introduce a modern information and communication system in endocrinological institutions, and to implement mutually integrated activities of endocrinological institutions. ... Along with this, it is necessary to ensure the timely allocation of funds for measures directed from local budgets to prevent endocrinological diseases among the population.
As the deputies noted, 52.75 of the 724.5 staffing positions allocated for endocrinologists in the republic are vacant, there is a need for their staffing, disproportion in providing the regions with medical places, it is necessary to strengthen work to create an electronic database covering all territories of the republic patients with endocrine pathologies. Among doctors who improved their qualifications abroad, the share of doctors from the regions is only 27 percent.
People’s representatives drew attention to the fact that the necessary funds for medicines and reagents, the allocation of which was determined at the expense of the local budget, except for Bukhara, Samarkand, Surkhandarya, Syrdarya, Namangan and Navoi regions, were not allocated in other territories, and in the Tashkent region - funds allocated only for iodine preparations. It was noted that endocrinologists have to work beyond what was planned, in this connection, proposals were made to introduce an additional position of an endocrinologist, including a pediatric endocrinologist, into the standard standards of primary medical and social care.
Based on the results of the hearing, taking into account the proposals and recommendations made, an appropriate decision of the committee was made.
